RELEASE OF LIABILITY & STUDIO RULES

RELEASE OF LIABILITY

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Gaby Lucas- House of Dance classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Gaby Lucas- House of Dance, its owners, volunteers, assistants, employees, guest artists, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted Gaby Lucas- House of Dance. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Gaby Lucas- House of Dance, its owners, volunteers, assistants, employees, guest artists, and/or students liable for such damage, loss, injury, or death.  I understand that I should be aware of my physical limitations and agree not to exceed them. 
By signing this agreement, I also acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren), family, party and I may be exposed to or infected by COVID-19 by attending HOD and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at HOD may result from the actions, omissions, or negligence of myself and others, including, but not limited to, HOD employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself or party (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at HOD or participation in HOD programming (“Claims”). On my behalf, and on behalf of my children, family and party, I hereby release, covenant not to sue, discharge, and hold harmless HOD, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of HOD, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any HOD program, rehearsal or recital.

 


STUDIO RULES- 2022-2023

HOLIDAYS & ATTENDANCE
“House of Dance” (the Studio) will honor Major HOLIDAYS as well as the Olathe School District Holiday Schedule.  We will also close when Schools close due to BAD WEATHER. 
ATTENDANCE... is very important to your child's progress; however, the Studio is not responsible for classes missed due to sickness or bad weather.  Make-up classes will be assigned or your child can attend a class of the same age ability.​ 
SUMMER 2022 session runs June 20th- July 28th
FALL-SPRING 2022-2023 session runs September 6th- May 19th

TUITION 
There is a $25 enrollment fee per student ($35 per family) for our Fall-Spring session 2022-2023.
Class fees are individually shown in our class calendar. 
Tuition is due by the 1st day of each month and can be paid by check (payable to HOUSE OF DANCE), 
or by Credit Card on-line.
Student Cap Discount: The maximum amount that can be charged per student is $235, regardless of how many classes he/she is enrolled in.

Credit Card Policy: All families enrolled must have a Credit Card associated with their account. Credit Cards will be automatically charged if balance is not paid by the 5th day of every month. When writing a check, please make sure that you write the CHILD’S NAME that you are paying for on your check so that it is properly credited to the right account. Payments can be handed directly to Gaby Lucas (or staff member) or mailed to: Gaby Lucas Inc, 16160 S. Heatherwood St. Olathe, KS. 66062. 
Receipts are available upon request.
Every student must have a Liability Waiver signed online by parent in order to participate in dance activities.


STUDENT AND PARENT GUIDELINES
*No student, family member or friend should use foul language, suggestions, or negative behavior of any kind around any of our Students, Parents or Instructors.
*No Food, Gum, and drinks are allowed in the dance studio classroom. (Bottled water is the only exception).
*Please properly dispose of all trash or food wrappers BEFORE entering the studio.
*Students should place their name inside all shoes and dance bags.
*Students are asked not to leave or walk out of the dance studio without asking permission. The teacher needs to know where you are at all times.


DRESS CODE
*Tiny-Combo: Dance tights, leotards, ballet slippers (NO TIES, please) tap shoes & hair pulled up and away from face.
*Mini-Combo: Tights, or leggings, or dance-shorts (tights must be worn under shorts);leotards or fitted tank tops (no midriffs showing);  jazz shoes, tap shoes; & hair pulled up and away from face. 
*Hip-Hop: Any type of clothing that is modest (no midriffs showing), does not restrict movement and that shows your own style! Clean tennis-shoes. (Please no rock or mud covered shoes).
*Jazz Technique Level 1: Tights/leggings, or dance-shorts (tights must be worn under shorts); leotards or fitted tank tops (no midriffs showing);  jazz shoes, tap shoes; & hair pulled up and away from face.
*Jazz & Lyrical Technique: Dance tights/jazz pants/dance shorts; leotards or fitted tank tops (no midriffs showing); split sole jazz shoes, & hair pulled up and away from face.
*Acro-Skills: Leotard, tights or dance shorts. Bare feet. No baggy clothing. 
*Ballet: Leotard, tights, skirts are optional. Hair pulled into tight bun.

 

BAD WEATHER/ TORNADOES/ OTHER EMERGENCIES
In case of bad weather, tornadoes or other emergencies we will take shelter inside our 2 bathrooms. 
Parents or guardians will be called in case of early pick-ups need to be made.
